PROGRAM EVALUATION REMINDERS

We appreciate and welcome your feedback as we continually evaluate our program services. One way you can help us is by completing the program evaluations that are sent to you when your match turns six months old and annually thereafter. This information is invaluable to your Program Specialist so they can hear about how your match is progressing and offer any support that you need.
