Before you begin the enrollment process, see if you can answer “yes” to the following:

All check boxes are required to continue.

  Yes   I am able to meet with a child 2 - 4 times per month for at least 6 months.
  Yes   I am at least 18 years of age.
  Yes   I have resided in Arizona for at least three months.
  Yes   I have an Arizona driver's license and valid auto insurance.
                 (exceptions are made for students and military personnel)
  Yes   I have not been convicted of a DUI in the past 5 years.
